Participant Waiver and Release of Liability(Required)

Please read and sign the document below. Without this document being agreed to your child will not be able to take part in any activities at Star Spirit

1. Acknowledgment and Assumption of Risk

I, the undersigned parent/guardian, understand that my child will be participating in activities during the birthday party at Star Spirit, which may include cheerleading, dance, tumbling, and/or aerial arts. I acknowledge that these activities involve inherent risks, including but not limited to sprains, strains, cuts, bruises, fractures, and, in rare cases, more serious injuries. While Star Spirit provides supervision, safety guidelines, and proper instruction, I understand that these risks cannot be entirely eliminated.

2. Medical Clearance

I confirm that my child is in good health and does not have any medical conditions that would prevent safe participation in the party activities. If I was unsure about my child’s ability to participate, I have consulted a physician and received clearance.

3. Personal Responsibility

I agree to remind my child to follow all instructions and safety rules provided by Star Spirit staff and instructors during the party. I understand that failure to adhere to these guidelines may increase the risk of injury. I will also ensure my child knows to inform staff immediately if they experience any discomfort, injury, or other concerns during the party.

4. Waiver and Release

In consideration of my child’s participation in the birthday party activities, I hereby release, waive, and discharge Star Spirit, its owners, employees, instructors, and agents from any and all liability for injury, loss, or damage to my child or property, except where such injury, loss, or damage is caused by negligence on the part of Star Spirit or its staff.

5. Emergency Medical Treatment

I authorise Star Spirit to seek emergency medical treatment for my child if necessary, in the event of an accident, injury, or other urgent medical need during the birthday party.

6. Photography and Media Release

I grant permission for Star Spirit to capture and use photos or videos taken during the birthday party for promotional and marketing purposes. I understand that I will not receive compensation for these materials. If I prefer my child not appear in such materials, I will notify Star Spirit in writing via email at hello@ before the party.

I have read and understood this waiver and release of liability. I understand that by signing this document, I am giving up certain legal rights. I sign it voluntarily and with full knowledge of its significance.
